I completely understand and realize that participation in the aforementioned event could include actions which might be hazardous or dangerous to my health including death.
By signing below, I agree to the fact that participation can cause harm or injury to me. I release, Your Mortgage Source and all related parties, from all liability, costs and damages which could arise from participation in the aforementioned event.
I agree to accept financial responsibility for the costs related to emergency treatment and/or injuries that may arise from participation in the event and give my confirmation of the same by signing this document. If under the age of 19 I will also need a parent or guardian to also sign the waiver.
RELEASE AND INDEMNITY:
I, the participant, on behalf of myself, members of my family, my heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless Alex Kotai, Your Mortgage Source and his representatives and agents for any injury, loss or damage to my person or property howsoever caused, arising out of or in connection with my taking part in “YMS $10,000 PRO AM Charity Hockey Tournament” and notwithstanding that the same may have been contributed to or occasioned by the negligence of the above listed parties or their representative or agents.
I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to myself or my family, including but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I or my family may experience or incur in connection with my attendance at The Rinks at Summit Centre.
On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless The Rinks at Summit Centre, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto.
There will be photographs and video taken at the event which may be used for promotional material by Your Mortgage Source. I acknowledge that Your Mortgage Source has the right to use these pictures/videos in all forms and in all media and in all manners, without any restriction as to changes or alterations (including but not limited to composite or distorted representations or derivative works made in any medium) for sale, advertising, trade, promotion, exhibition, or any other lawful purposes, and I waive any right to inspect or approve the photograph(s) or finished version(s) incorporating the photograph(s), including written copy that may be created and appear in connection therewith.
